I don't know what church this is, but I believe it's Lutheran or Reform. The poem is a variant of a hymn by Johann J. Rambach, 1693-1735 Ich bin getauft auf deinen Namen (English = "Baptized Into Thy Name Most Holy") which is in the Lutheran "Evangelical Songbook".

This is probably also in Hellertown, Northampton County, Pennsylvania. The minister is Samuel Hess.

The minister is M. Loy or Soy.
